Grammar issues

I complain about grammar issues all the time. How can fluent and natural speaker of English speak so poorly?

Example A: I am on an Amtrak train right now and occasionally we come to a stop at which some of the train’s doors will not open. Without fail, the conductor says “all doors will not open at [fill in stop name]“. Why are we stopping then? At least one door should open so people can get off. I imagine of course that they mean to say “not all doors will open at …”. And they do it every time.

Example B: My Yale-educated friend told me “the subway will be equally as fast as a taxi”. Maybe he meant “equally fast”, or maybe he meant “as fast”.

It reminds me of a West Wing episode where a writer says “extremely unique”.
